Problem
CareLoop addresses a fundamental limitation in psychosis care: the inability to reliably detect early deterioration before relapse and crisis occur. Psychosis is a chronic, episodic condition where relapse is common and often preceded by subtle symptom changes over a short prodromal period (3-10 days). However, traditional care models rely on infrequent clinical contact, meaning early warning signs frequently emerge between appointments and go undetected.
As a result, deterioration is often recognised only once individuals present in crisis, requiring urgent intervention, inpatient admission, or intensive community support. Relapse is associated with increased clinical risk, poorer long-term outcomes, and cumulative harm to individuals. It is also a major driver of prolonged inpatient stays, bed occupancy, and out-of-area placements.
This creates sustained operational and financial pressure on NHS mental health services. Existing digital investment has largely focused on infrastructure and access, rather than enabling continuous clinical visibility and proactive intervention. This leaves services without a scalable mechanism to systematically identify and respond to early deterioration, limiting their ability to deliver preventative, community-based care.
Solution
CareLoop provides a clinically validated remote monitoring platform that enables continuous, personalised detection of early warning signs of deterioration in people living with psychosis. Patients use a smartphone app to complete brief, structured symptom check-ins based on validated clinical PROMs questionnaires. CareLoop’s algorithm analyses this data to establish an individual baseline and identify meaningful deviations that indicate increased risk of relapse.
This creates continuous visibility between routine clinical contacts, allowing early warning signs to be identified during the prodromal phase when intervention is most effective. Alerts provide timely insight to enable appropriate review and intervention, supporting earlier, more proactive care.
By systematically identifying deterioration earlier, CareLoop enables services to intervene before escalation to crisis. This helps prevent avoidable relapse, reduces inpatient admissions and bed use, and supports safer care in the community.
The platform provides a scalable and practical mechanism to operationalise preventative care, improving outcomes for patients while reducing avoidable demand on NHS services and supporting more sustainable mental health care delivery.
Impact
In research in the Empower study published in the Lancet Psychiatry, CareLoop as an adjunct to standard care within a CMHT setting was seen to reduce incidence of relapse by 50% vs standard care alone and a deliver a 33% reduction in hospital costs.
The evaluation and data from the pilot phase across 5 NHS Trusts (Devon Partnership NHS Trust, Cornwall Partnership NHS Trust, Livewell Southwest, Greater Manchester Mental Health NHS FT, Leeds and York Partnership NHS Foundation Trust) will be published in due course.
